Charlie Begley died at Royal Victoria Hospital in Belfast on January 23 at age 55 following a transfusion of incompatible blood two months earlier. His partner David O'Prey, 66, from south Belfast, demands answers on the error. The couple had been together 36 years and married eight years. Both previously worked at a Belfast hospital.

O'Prey holds the health trust responsible for shortening Begley's life. He expressed fears Begley voiced days before death about not surviving. O'Prey insists those involved face consequences to prevent repeats.

Belfast Health and Social Care Trust declared a Level Two Serious Adverse Incident. The trust notified authorities over administration of incorrect blood product in Begley's care. An investigation continues.
